The 1815 Project
Improving the Life of South ÌÇÐÄÊÓÆµns through the
Prevention and Management of Diabetes and Cardiovascular Disease
In spring 2018, the Centers for Disease Control released a call to action to address health disparities among Americans with diabetes, heart disease and stroke through CDC-1815. In response, the South ÌÇÐÄÊÓÆµ State University College of Pharmacy and Allied Health Professions and the South ÌÇÐÄÊÓÆµ Department of Health collaborated to create a five-year plan to identify barriers and develop viable solutions to improve the care of South ÌÇÐÄÊÓÆµns with diabetes and cardiovascular disease. Over the five years of the project, grants totaling over $2.5 million were awarded to the South ÌÇÐÄÊÓÆµ State University College of Pharmacy and Allied Health Professions team to complete this work.
